Cole Vocational Services/First Step Independent Living/Unlimited Quest
With locations throughout southern California, Cole Vocational Services, First Step Independent Living and Unlimited Quest provide day habilitation and employment services to adults with developmental disabilities in both center-based and community-based programs. They joined The Network in December 1997.
Cornerstone Living Skills
Cornerstone Living Skills serves developmentally disabled children and young adults in nursing facilities located in Santa Rosa, California. These residences provide home-like settings and offer clients active involvement in the community. In June 2002, one residence became part of a California-based pilot program offering twenty-four hour continuous nursing services for those with more acute nursing needs. Cornerstone Living Skills joined The Network in June of 2005.
Creative Home Programs
Creative Home Programs, located in Riverside, California, operates an in-home early intervention program that serves over 400 infants and toddlers with disabilities or developmental delays as well as young children at risk for delay. Goals are individualized and focus on enhancing communication, sensory awareness, cognition, gross and fine motor skills, social development and adaptive living skills. Creative Home Programs also offers a specialized program for children diagnosed with autism spectrum disorders. The company joined The Network in November 1999.
Family Advocacy Services
Family Advocacy Services (FAS), of Maryland, is a community-based provider offering a continuum of educational, clinical and therapeutic placements for children and adolescents with emotional and behavioral challenges. Founded in 1986, FAS is headquartered in Baltimore, Maryland and serves youth in surrounding communities as well as southern Maryland and northern Virginia. FAS also operates several Alternative School programs including New Foundations of Baltimore and the Florence Bertell Academies of Baltimore and Prince George’s County. Within each school, they provide academic, clinical and vocational programming based on an individualized education plan that is developed annually for each student. The company joined The Network in December 2002.
IFCS (Institute for Family Centered Services)

IFCS serves families in Florida, Maryland, North Carolina and Virginia through intensive in-home family centered services. Founded in 1988, IFCS’ service model provides individualized, practical support to families, including assistance during times of crisis and help with parenting in unusual or difficult circumstances. IFCS also works with families to resolve conflicts, address their communication needs, and provide support when the tasks of daily living are overwhelming. IFCS joined The Network in 2009.
Loyd's Liberty Homes
Loyd's Liberty Homes operates programs for individuals with developmental disabilities in Fresno and Bakersfield, California. Operations include 26 intermediate care facility homes, two adult day programs, as well as supported living, supported employment, and crisis response programs. The company joined The Network in June 1998.
MENTOR
Founded in 1980 in Boston, MA, MENTOR is a leading national provider of home and community-based services to three special needs populations, including: Individuals of all ages with intellectual and/or developmental disabilities ("I/DD"); children and youth with who are behaviorally, emotionally and/or medically challenged (“At-Risk Youth”); and persons with acquired brain injuries (“ABI”). MENTOR offers an array of community-based residential and non-residential services that range in intensity, duration and frequency depending on the needs of the client. The Company’s core treatment foster care model (The MENTOR Model) is based on a one-to-one placement of a client in the home of a trained paraprofessional called a “Mentor.” Other service and support options include adoption assistance, educational services, in-home support, day programming, employment services, small group care and supported living. MENTOR provides services in more than 20 states.
NeuroRestorative

NeuroRestorative provides post–acute rehabilitation services for adults and children with brain and spinal cord injuries as well as other neurological challenges.
NeuroRestorative combines the legacies and resources of a select group of highly reputable providers, each of which has played a key role in shaping the field of post-acute rehabilitative services. In July of 2010, Network partners Lakeview, MENTOR Brain Injury Services, the Center for Comprehensive Services, and NeuroRestorative Specialty Centers were united under one brand, and today share the name NeuroRestorative.
Programs and supports are delivered in community-based settings, with an ongoing focus on participants’ well-being and advancement toward independent living.
Along with CareMeridian, NeuroRestorative represents The MENTOR Network's Post-Acute Specialty Rehabilitation Services.
RCDS

Rockland Child Development Services (RCDS) is an early intervention program under contract with New York City, Rockland County and Orange County to provide home-based evaluation and ongoing services for infants and toddlers experiencing developmental delays. RCDS, which joined The Network in 2006, is a dynamic program with a core team of dedicated clinicians and therapy providers in the field. RCDS has been in operation since 1999 and serves Rockland and Orange counties, Manhattan, Bronx, Brooklyn and Queens. For more information please visit the RCDS website at www.rcdseip.com.
REM
Since 1967, REM has been providing a variety of services and programs for people with special needs and their families. Each program is designed around the needs of the individual, coordinating their strengths with the supports available in the community. By providing a wide range of service options, and by adapting those services as the needs of the individual change, REM ensures a quality life experience for each person. Services include: Community Living Options; Vocational and Day Supports; Home Health Care; Homemakers Services; Early Intervention; Rehabilitation Therapy; Foster and Host Home Living Options; and Assisted Living for Seniors. REM became a partner of The MENTOR Network in May 2003.
TSI
Transitional Services, Inc.(TSI), based in Evansville, Indiana, has provided residential and supported living services to individuals with developmental disabilities since 1986. TSI operates group living residences and also provides a wide variety of other individualized residential and support services to individuals with disabilities who want to continue to live in their personal or family home. Support services available include in-home staffing support and skill training, community living and integration training, money management assistance, health care coordination, and adult and child foster care services. TSI joined The Network in August 2008.
Virginia NeuroCare
Virginia NeuroCare (LVN) in Charlottesville, VA, is one of two civilian-run programs in the U.S. that offer advanced rehabilitation to military patients with mild to moderate brain injuries. The program provides a community integrated setting for active rehabilitation, comprehensive evaluations, an outpatient therapy clinic, vocational training and as well as skill development activities and innovative community re-entry services. Virginia NeuroCare joined The Network in 2009.
Preferred-Provider Relationship: Alliance
In several states, MENTOR provides Therapeutic Foster Care as a subcontractor of Alliance Health and Human Services, a nonprofit organization based in Newton, Massachusetts. For more information please visit www.alliancehhs.org.
