CareMeridian

CareMeridian, one of The MENTOR Network’s partners located in California, Nevada and Arizona, is proud to announce that Village Healthcare Centers (VHC) has joined them in California.

Similar to the services offered by CareMeridian, Village Healthcare Centers provides long- to life-term care and supports to adults who have neurologic disorders or ventilator dependency, or who have sustained traumatic brain or spinal cord injuries.  VHC has operated in California since 2001, and offers specialized care to the individuals they support, including skilled nursing, rehabilitation, ventilators, tracheostomy, intravenous and other forms of complex care services.

“We are pleased to welcome the patients and staff of Village Healthcare to CareMeridian,” said Jim Ashby, CareMeridian CEO and President.  “The services offered by VHC align perfectly with the supports provided by CareMeridian, and we are excited to have the opportunity to expand our services to additional individuals.”

CareMeridian provides sophisticated sub-acute care alternatives for catastrophically ill or injured patients of all ages.  With 14 locations located throughout Northern, Central and Southern California, as well as in the southwest locations of Las Vegas, Nevada, and Phoenix, Arizona, CareMeridian provides a unique service delivery model, merging the aesthetics of a residential environment with the framework of state-of-the-art technology and personalized service from an experienced and skilled health care team.

Village Healthcare Centers serves up to 12 individuals in its Santa Ana, California location.  A new Garden Grove location – which is currently under agreement and expected to join CareMeridian in the next few weeks – will have the capacity to provide services to an additional 15 individuals.  The services and supports currently provided by VHC will continue without interruption as operations transition to CareMeridian.

“CareMeridian has a strong history in the community for providing exceptional care to individuals with highly specialized needs,” said Dan Larson, CareMeridian’s Chief Development Officer.  “The expertise and vision that the staff at VHC brings with them are closely aligned with the mission of CareMeridian, and this partnership can only serve to strengthen the quality of the services we provide.”
