Our specialized rehabilitation services—delivered by our partners, CareMeridian and NeuroRestorative in community-based settings—offer a broad range of medical care, therapy, training, community integration and behavior management that help each person move through their rehabilitative journey. As part of The Network’s Specialty Rehabilitation Services group, CareMeridian and NeuroRestorative are committed to delivering powerful outcomes for those we serve. While both offer a range of treatment options, each partner also offers specialized services and supports that are unique to those they serve.
As a leader in subacute care, CareMeridian’s specialty rehabilitation and nursing facilities throughout the Western United States offer 1:4 patient ratios and 24/7 nursing care to serve individuals with a wide range of medically complex challenges, including brain and spinal cord injury, neuromuscular disorders, pulmonary disease, burns or wounds.
NeuroRestorative offers services in more than 20 states and specializes in community-based, post-acute brain injury rehabilitation services, including Neurorehabilitation, Neurobehavioral, transitional living, supported living, and outpatient services. Collectively, NeuroRestorative’s services and supports promote community involvement and help adults become more independent.
Services for Veterans & Military Service Members
The MENTOR Network is also honored to serve Veterans, Military Service Members and their families. Through our partners, CareMeridian and NeuroRestorative, we offer a range of inpatient and community-based service options for individuals with brain and spinal cord injuries or other complex medical or neurological challenges. Through an understanding of military culture—honed from years of experience serving America’s heroes—we focus on creating individualized service plans to ensure we meet each person’s physical, cognitive, behavioral, and emotional needs.
